Spectated a match which was in progress on turn 3. When the match was completed, The attacking Unchained Aruha disappeared from the field, as well as the "player 1" cards in hand grave and field, but Player 2's other cards on field remained. Abominable Unchained Soul and Unchained Soul of Rage remained visible on the field as well as their grave after siding was complete. This was cleared by leaving the match and rejoining.]

Player 1's life points were at 2000. "Opponent's" Abominable Unchained Soul attacked into a face-up defense position Ash Blossom & Joyous Spring, Unchained Soul of Rage attacked directly, and Unchained Aruha attacked directly for game. The game stayed on the field and siding began for players. Match 2 began and the opponent still had "monsters" showing on field.]

Notice that this is the first turn of the game, and the Dino Player is going first. There is no way for the opponent to achieve this board state “legally” during player 1’s turn. Once left from the game as a spectator, the game state was refreshed.
Once siding was complete, the board should have been wiped clear to start the new game.]
Also just noticed this as well: The player going first did not have the number of cards reduce when summoning a monster from their hand.
